CONTROLLED SUBSTANCES

Please be aware of our controlled substance policies outlined below. We DO NOT manage long-term pain patients. The controlled substance agreement is available below for your review. 

Medication Policy

We reserve the right to turn away or deny new patients at our discretion. Urine drug screens are required prior to initiating any new controlled prescriptions. 

Appointments

We require patients to schedule their next appointment prior to leaving the office. It is not the responsibility of office staff to manage appointments for patients to ensure that there is not a lapse in prescriptions. We do not provide short-term refills of controlled substances. 

Discharge

We reserve the right to discharge controlled substance patients at our discretion. Medication mismanagement, self-acceleration, multiple missed appointments, and violations of our controlled substance agreement are all valid reasons for discharge.
